Renal dysfunction caused by acute Nephritis
At the early stage of onset, the urine volume may decrease due to decreased glomerular filtration rate and retention of sodium and water, and even oliguria ( < 400 ml / d ) may occur in a few patients. Renal function can be temporarily impaired, with mild azotemia. After more than 1 - 2 weeks, urine volume gradually increased, and renal function gradually returned to normal days after diuresis. Only a few patients can show acute renal failure and need to be differentiated from advanced nephritis.
Congestive heart failure
It often occurs in the acute phase, and serious retention of water and sodium and hypertension are important inducements that need urgent treatment.
Abnormal immunological examination
The transient decrease of complement C3 in serum: More than 2 weeks after the onset of the disease, it decreased and gradually returned to normal within 8 weeks, which is of great significance to the diagnosis of the disease. The titer of anti - streptococcal hemolysin " O" in the patient's serum can be increased.
Treatment of acute nephritis
1. General treatment
At the acute stage, the patient should stay in bed and gradually increase his activity after the hematuria disappears, edema subsides and blood pressure returns to normal. In the acute phase the patient should be supplied with low-salt ( below 3g daily ) diet. People with normal renal function do not need to limit protein intake, but when azotemia occurs, protein intake should be limited and high-quality animal protein should be the main component. Patients with acute renal failure with obvious oliguria need to limit the amount of liquid intake.
2. Treatment of infected foci
If the infection exists, treat it accordingly.
3. symptomatic treatment
Including diuresis, detumescence, lowering blood pressure and preventing heart and brain complications. When the control of hypertension is still unsatisfactory after rest, low salt and diuresis, antihypertensive drugs can be added.
4. dialysis treatment
A few patients with acute renal failure who have dialysis indications should receive timely dialysis treatment to help them survive the acute phase.
